Terms of Service

These terms are an agreement between you and [BUSINESS NAME] ("we", "us"), the provider of FillOnce. By creating an account or using FillOnce, you agree to them. If you use FillOnce for a practice, you confirm you're authorized to agree on its behalf.

1. The service

FillOnce is a web application that stores practice and provider credentialing information and helps you complete forms, track enrollments, supplies and expiration dates, and prepare letters. We may improve or change features over time.

2. Accounts

You're responsible for keeping your password safe, for everyone you give access to your practice, and for removing access when someone leaves. Tell us right away at [EMAIL] if you think your account has been misused.

3. Subscription, trial and billing

Each practice gets a 14-day free trial (one per account). After that, FillOnce costs $199 per practice per month, plus $49 per month for each additional office location, billed in advance through our payment processor, Stripe, until you cancel. Prices may change with at least 30 days' notice. You can cancel any time from Manage billing; access continues until the end of the paid period, and we don't give refunds for partial months except where required by law. If a payment fails or a subscription ends, your data remains viewable but editing pauses until payment resumes.

4. Your data

You own the information you enter. You give us permission to store and process it only to provide FillOnce to you. You're responsible for the accuracy of what you enter and for reviewing every form, application and letter before you submit or send it. FillOnce is a productivity tool; it does not submit applications on your behalf and does not guarantee any credentialing or enrollment outcome.

5. No patient information

FillOnce is not designed to store protected health information (PHI). You agree not to enter patient records or other PHI into saved fields. We do not sign Business Associate Agreements. Patient details typed into the Letters screen are not saved.

6. Acceptable use

Don't misuse FillOnce: no attempts to access other practices' data, disrupt the service, reverse-engineer it, or use it for anything unlawful.

7. Ending your account

You can delete a practice (after cancelling its subscription), which permanently deletes its data. We may suspend accounts that break these terms or are unpaid for more than [60] days, after notice where practical. We may delete data from practices that have been unpaid for more than [12 months], after notice by email.

8. Disclaimers

FillOnce is provided "as is." We work to keep it available and accurate, but we don't promise it will be uninterrupted or error-free, or that expiration alerts will catch every deadline. You remain responsible for your credentialing obligations.

9. Limitation of liability

To the extent the law allows, we aren't liable for indirect, incidental or consequential damages, or lost profits or revenue. Our total liability for any claim is limited to the amount you paid us in the 12 months before the claim.

10. Changes and governing law

We may update these terms and will notify you of material changes by email or in the app. These terms are governed by the laws of the State of [STATE].
